  • Subarachnoid hemorrhage needs to be on your differential for syncope but keep in mind that neuroimaging is only indicated if the H&P suggests (pt on warfarin, >65yo, or possible trauma).
  • Black widow spider bite? Think:
    1. They produce a neurotoxin (latrotoxin).
    2. Centrally-spreading pain that eventually causes crampy abdominal/chest symptoms.
    3. Narcotics and benzodiazepines are first-line therapies
    4. Consider rhabdomyolisis.
    5. Antivenin indicated in refractory cases.
  • Five places that you can exsanguinate:
    1.  Thorax
    2. Peritoneum
    3. Retroperitoneum
    4. Femurs
    5. The Floor
